Does anybody have the parents fill out an evalution of the teachers? If you do, please post the questions that you have on the evaluation.

We put out regular parent feedback forms with the usual;
Do you find staff approachable? How do you feel staff respect your veiws as parents? What do you like about the environment? What don't you like about the environment? What do you like about the program? What don't you like about the program? Do you feel able to contribute to the program? How do you feel you could contribute to the program? What policy do you not like at the centre? What policy would you like to see introduced to the centre? How could you feel more a part of the centre?
I always leave a section for overall comments and gripes and leave names out, i have found even making them optional tends to invite insencere praise. I hope this helps
