i need to make a feedback sheet to parents about our programme, but im not sure of what kind of questions i could ask, anyone have any suggestions??

Maybe list the choices for these questions as Never, Somewhat, Usually, and Always?

1. I feel welcome when I visit this school.

2. This school provides a safe environment for learning.

3. The school quickly informs me about problems and needs of my child.

4. School personnel listen to and welcome my ideas about how to improve the school.

5. Up-to-date instructional tools (books, computers, videos, etc.) are used effectively in the school.

6. The school provides instructional programs that challenge my child.

7. My child's teacher(s) communicate effectively with me.

8. Administrators in this school are available for parent conferences when needed.

9. I receive regular progress reports from my child's teachers and other service providers.

10. Students and parents are respected and treated fairly.

11. School grounds, buildings and classrooms are clean and well maintained.

12. My child has a postive attitude about school.

13. Student discipline is handled appropriately.

14. I am involved at my child's school by attending parent conferences, open house, and other functions.

15. I regularly read newsletters and other notices sent home from school.

16. The school handbook adequately answers all of my questions.

Great ideas for a handout Mickey! I think most of the parents in my class would have to mark "NO" or "NEVER HEARD OF THE CONCEPT" to # 15.

I agree with Ryleigh. Most of my parents would have to mark no to 15 too. Then they wander why they don't know what is going on. I usually tell them, oh you must have missed that in the newsletter. They usually look at me like I am stupid.
